Original I-94 (the white card stapled inside your passport) or I-94 printout obtained at: https://i94.cbp.dhs.gov/I94/#/home
Financial document: more than 15,000 USD of Bank Statement
a. If BK Statement is not student's, Affidavit of Sponsor is required.
b. If visa holder has dependent, Additional 3,500 USD per dependent for an year of I-20
A letter requesting reinstatement to F-1 student status which explains your situation and truthfully states that failure to maintain status was due to any or all of the reasons listed above
Supporting documentation (for example, a letter of support from a faculty adviser familiar with the circumstances of your case)
Current transcript and transcripts from other U.S. schools previously attended
Make a payment - 100 USD / Non-refundable
Receive an I-20 & Acceptance Letter from school and pay for SEVIS Fee
*You must pay a new SEVIS fee when filing reinstatement. The fee can be paid online at www.fmjfee.com.
Submit your case to USCIS with Form I-539

The $370 Fee can be paid in the online form once you file to USCIS after we give you the reinstatement I-20 or
If you file by mail, you will need a check or money order made payable to "U.S. Department of Homeland Security" in the amount of $370
You will receive a receipt notice from USCIS within about 2 to 4 weeks. USCIS will take approximately 4 to 12 months to process your application. Once a decision is made regarding your reinstatement, you will receive a letter in the mail from USCIS.
